Why this structure
The letter does three things in order, because that is the order a housing provider's own review follows. It makes the request and names the federal duty (42 U.S.C. § 3604(f)(3)(B)). It answers the documentation question before it's asked: a letter from a licensed professional is what 24 C.F.R. § 100.204 contemplates, and you don't owe anyone a diagnosis. And it preempts the money objection: once an accommodation is granted, the animal isn't a pet, so pet rent, deposits, and breed rules stop applying to it. Does sending this letter guarantee the accommodation?
No. The letter makes the request properly (it asserts the right legal framework and attaches the right documentation), but the accommodation exists because a licensed mental-health professional determined a disability-related need, not because a form was filled in well. A housing provider may verify the documentation, and a specific animal that poses a direct threat can still be refused.

How should I send it?
Any channel you can date and keep a copy of. Email is fine for most landlords; if you already expect a dispute, certified mail gives you proof of receipt. What matters is that the request is in writing and you can show when it was made.
What if the landlord refuses or ignores the request?
A refusal needs a lawful ground, and silence is not one. An accommodation request stays live while a documentation question is being resolved.
